Eye Print Waiver of Liability & Release Form Adult/Minor
ACKNOWLEDGEMENT
I (the “Client”) hereby authorize Eye Print LLC & MyEyePrint.com to photograph my eye, specifically either one or both of my irises, and digitally process and print images for display as art for your purchase. Unless you specifically authorize EyePrint to retain and display or publish for promotional purposes an image of your iris, the digital files will not be kept for a period longer than 30 days before the file is permanently destroyed, ensuring your privacy and your exclusive enjoyment of the image.
I hereby release and hold harmless Eye Print & MyEyePrint.com from any reasonable expectation of privacy or confidentiality for myself and/or any minor child and/or children listed below associated with the images specified above.
You hereby acknowledge and agree to hold harmless Eye Print & MyEyePrint.com from all liability to the fullest extent allowed by law. You are allowing Eye Print LLC & MyEyePrint.com to take detailed photographs of your eye which entails shining a light into your eye which may be momentarily irritating. Please note that we are not medical professionals and do no screening for health conditions. We maintain high standards of hygiene but urge that all precautions are taken to avoid contracting communicable diseases given that Covid remains a known risk. We reserve the right to refuse service to any customer for any reasons at our own discretion.
I further acknowledge that participation is voluntary and that neither I and/or the minor child or children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in Eye Print LLC & MyEyePrint.com marketing materials or other publications.
I acknowledge and agree that publication of said photos confers no rights of copyright or royalties whatsoever, which rights shall be exclusively retained by Eye Print LLC& MyEyePrint.com. I hereby release MyEyePrint.com, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation and/or the minor children listed below.
IMPORTANT NOTICE REGARDING COPYRIGHT: It is illegal to copy or reproduce the Clients photographs elsewhere without Eye Print LLC & MyEyePrint.com’s permission, and violators of this Federal Law will be subject to its civil and criminal penalties. All photographs, both on MyEyePrint.com’s website, blog or Facebook page, including all potential proofs and purchased prints, are copyrighted, reproduction of any image by any means, including copying, re-printing, screen captures, and other means of print reproduction are strictly prohibited and are a violation of copyright law. Additionally, it is illegal to post any of MyEyePrint.com’s work on the Internet, blogs, and social networking sites, without the express permission of the photographer. The only exception to this is if you have purchased the print release digital files provided by MyEyePrint.com & Eye Print LLC.
Photo processing times vary and you understand that it can take between 10-30 minutes depending on client traffic.
